Dr. Render Parkman
Dentist – both locations
Dr. Render Parkman is originally from Sylvester, Georgia. He graduated magna cum laude from Valdosta State University, where he was also a member of the baseball team. Dr. Parkman earned his D.M.D.from the University of Louisville School of Dentistry, where he was very involved in the local chapter of the American Student Dental Association and served as the student body president. Dr. Parkman was given the undergraduate award from the Academy of Osseointegration, an international dental implant organization. Following graduation, he served as a captain in the United States Air Force, completing a one-year Advanced Education in General Dentistry residency in Louisiana. He completed the remainder of his Air Force requirements in South Carolina. Dr. Parkman has completed advanced training in intravenous sedation techniques, oral surgery, implant placement, and periodontal surgery. Dr. Parkman loves golf and rooting for University of Georgia football and Louisville basketball. The Parkmans recently joined Southeast Christian church and look forward to serving in whatever capacity they are needed!
Dr. Laura Parkman
Dentist – both locations
Dr. Laura Parkman is originally from Lebanon, Kentucky. She graduated magna cum laude from Campbellsville University, where she played varsity soccer for four years. Her collegiate soccer career gave her the opportunity to travel abroad on mission to Germany, Ireland, and Northern Africa. Dr. Parkman then earned her D.M.D. from the University of Louisville School of Dentistry, where she also served as student body president and president of the Louisville Chapter of the American Student Dental Association. Upon graduation, Dr. Parkman was given the undergraduate award from the International Congress of Oral Implantologists. Following her graduation from UofL, Drs. Render and Laura Parkman were married in Louisville and she has since worked in private practice. She has done continuing education in endodontics, pediatric dentistry, removable prosthetics, and implantology. Dr. Parkman loves traveling, reading, UofL sports, and hopes to someday take dental mission trips to third-world countries. Render & Laura welcomed their first child in December 2014, Lilly Claire, who is full of spunk and has a contagious smile!
Brittany Simpson

Brittany grew up in Madisonville, Kentucky. She attended West Kentucky Technical College in Paducah and graduated the Expanded Duties Dental Assisting Program in 2015. She loves being a part of creating new smiles and giving people back their confidence. She has a passion for photography, traveling, being outdoors with Leo her German Shepherd, and spending time with friends and family.
Jenny Forsyth
Registered Dental Hygienist
Jenny is originally from Bloomington, Indiana, but attended secondary school in Floyds Knobs, Indiana. She graduated magna cum laude from University of Louisville School of Dental Hygiene and is licensed to practice dental hygiene and local anesthesia by the Kentucky Board of Dentistry. Prior to dental hygiene school, Jenny earned a Bachelor of Science in Biology from Indiana University and was employed throughout college as a dental assistant. Jenny's strong educational background and work experience has allowed her to become a very skilled clinician with a great understanding of periodontal treatment and care. In her free time, Jenny enjoys traveling, cooking, and cheering for her Indiana Hoosiers. Her goal is to visit all 50 US states and dreams of traveling to Europe; but Jenny's favorite place to vacation is Connecticut where her niece and nephew live. Jenny takes great pride in honesty, providing superior treatment, and creating personal relationships with her patients. We are excited to have her on the Foundations team!
Dr. Carl Kuhl
Dentist
Dr. Kuhl is a native Louisvillian. He attended Georgetown College on an art scholarship and graduated from there in 1975 with a degree in Art, and a minor in Chemistry. Carl taught at Highland Junior High for one year and worked toward a PhD in anatomy before enrolling at UofL's dental school. He graduated from UL school of dentistry in 1981 and started practicing in St. Matthews shortly after. He took over the practice location at 3618 Lexington in 1983. Dr. Kuhl feels fortunate to have been able to meet so many wonderful patients over the years and share in their lives. In his spare time, Dr. Kuhl loves spending time with his 11 grandchildren, who are spread from coast to coast in the continental U.S. When he is not traveling to see them, he enjoys theater, music, and oil painting.
Miranda Ashley
Hygienist
Miranda grew up in Springfield, KY where she worked as a dental assistant while attending dental hygiene school. After graduating from Lexington Community College (2000), she began her practice here at Foundations Family Dental (formerly Carl Kuhl DMD). Many years later, she strives each day to know each patient's story and give them the best care possible. In her time away from the office, Miranda enjoys spending time with her husband, Craig and step children, Parker and Madison. They have been on mission trips from the Hills of WV to the African Country of Uganda and plan to take many more. While not traveling, Miranda enjoys running, reading books, experiencing new restaurants, and serving in the children's ministry at her church.
Cindy Gallahue
Assistant
Cindy grew up and lived in St. Matthews all her life. She graduated from Jefferson Vocational School for dental assisting and later obtained her Expanded Duties Certification from University of Louisville in 2002. She married her high school sweetheart and has 2 grown children and 4 grandchildren. The lake and boating are some of her favorite hobbies. She loves and enjoys her family and considers the practice part of her extended family.
Sarah Black
Front Desk
Sarah joined our team in July 2017. She has previous experience as an Insurance Coordinator in a dentistry office in West Virginia & Virginia. Sarah attended Marshall University and has a Bachelor’s Degree in Business Administration. Her outgoing personality and willingness to help are outstanding. She enjoys traveling, camping, working out and taking walks with her Great Dane Calla.
Destinée Fleener
EDDA
Destinee is from and currently resides in Shepherdsville, KY. She graduated Summa Cum Laude from Medquest college in April of 2019. She has a 2 year old son named Elijah who is her little miracle baby. She enjoys creating and making things with vinyl and being outdoors with her son, family and friends. She has had many different roles in the health care field and after searching, it was an easy choice to choose dentistry. She enjoys what she does daily, knowing that at the end of the day, she played a role in helping brighten someone's smile.
